Incident Info
Disclaimer
NERIS is currently under development by the Fire Safety Research Institute (FSRI). This guide reflects the system as implemented in EPR FireWorks. Functionality may evolve as NERIS continues development toward full national implementation by January 2026.
Recording Incident Info
The Incident Info tab of the NERIS report includes all the basic details of the incident, including signatures and authorization of the report.
Complete incident documentation by addressing each required section:
General: Basic incident identification and response details
Incident Types: Incident categorization according to NERIS standards (three required per incident)
Actions & Tactics: Response actions and tactical operations deployed
Address & Location: Incident location and geographic data
Involved Entities: People, organizations, and resources involved
MVC/Vehicle Info: Motor vehicle collision details (where applicable)
Custom Fields: Department-specific data elements
Narrative: Detailed incident description and operational summary
Authorization: Report approval and submission signatures

Completing Incident General Details
Document basic incident identification and response details in the General section.

Note: Select preset NERIS scenarios to streamline incident documentation with pre-configured field values. Click Choose Scenario to select a preset (e.g., Structure Fire, Medical Incident, MVC). The system automatically populates applicable fields specific to that incident type, reducing data entry time and ensuring consistency.

Complete the following fields:
Internal Incident #: Department-assigned incident identifier
Station: Responding station assignment
Shift: Active shift designation
Risk Category: Internal department classification for incident severity
Population Density: Rural or Urban
Aid Given or Received: Document mutual aid responses.
Note: If this field is not set to Aid Given or Aid Received, you will not be able to document mutual aid agencies in the Response tab.Emergency/Non-Emergency: Designate incident priority classification.
FSRS Incident: Designate for Fire Service Resource Survey reporting.
CAD-integrated fields (Station, Shift, etc) auto-populate when available. Verify accuracy before submission.
